1 - First choose dimensions to make your polar co-ordinate, that is
2 - Match the two dimensions, choosing the bigger number (e.g 297, 297)
3 - Rotate the image so that it's upside down
4 - go to Filter>Distort> Polar Co-Ordinates
5 - Check it is circular ( not rectangular)
6 - Ready to go
